Thanks for choosing Body Evolution Studios! We require all class participants sign a waiver before being allowed to participate in sessions.
To fill out our digital waiver PDF, you will require Adobe Acrobat Reader. You can download it from Adobe’s official website. Click the yellow “Download Acrobat Reader” button.
After installation, open the PDF file that was linked in your class registration confirmation email in Acrobat Reader.
On the right hand side of the program screen, you will see a section of tools. Click “Fill & Sign”, which is highlighted in red in the above image.
When the above prompt shows, click “Fill and sign”, highlighted in red.
Using the Sign and Fill tool, point the cursor over the underlined areas next to “Printed Name” and “Date” to make the blue highlighted area appear (shown above). Fill in these fields using this. Then, Click the “Sign” tool to add your signature (shown below, highlighted in red)
This will bring up the signature panel, you will need to click “Sign” (Shown below, highlighted in red), and then use your mouse to draw and create a signature. When done, click apply.
This will allow you to place your signature. Place it in the field next to ‘SIGNATURE’
The final steps are to simply save the PDF, which can be done via File > Save As (shown below). Once saved, simply add the saved PDF as an attachment in an email to email@example.com.
Note: You must fill out the Printed Name and Date field before adding your signature, as adding a signature will not allow you to fill any fields out once it has been added.
